Epicondylar Musculature Repair CPT/DX?

How would you code the repair of a lateral epicondylar muscle rupture? Would that fall into the 24357 - 24359 range, or is there a different CPT code that better suits this procedure? Also, for the dx, would lateral epicondylitis be used? Any advice would be greatly appreciated, thank you.

OWCP

Has anyone been able to reach anyone at OWCP since they changed administrators? Every time I call I get a message saying due to call volume they are unable to take my call and I get disconnected. I never had this issue with Conduent. CNSI has been a nightmare.:mad: Approvals have stalled, claims aren't being processed, and although I have found some information on the portal its been like beating my head against a brick wall. I don't even know who to go to to try to get some resolution!

Need official reference for documentation of LT heart cath crossing aortic valve

Good morning,

I'm looking for an official reference to prove that documentation of the crossing of the aortic valve is necessary in order to code a left heart catheterization. Some at my facility have been told that as long as pressures are documented, a LT heart cath can be coded. In my experience, I have been told those pressures could be obtained on the aorta side of the aortic valve without the catheter traversing the aortic valve.

Billing 45385 and 45381 to Blue Cross Medicare packages

I really need some help!
Lately I am having difficulty getting 45381 paid when coded with 45385. I have sent supporting documentation as well as "clear Connect" screen shot from Web Dennis showing this code combination is allowable. On one of my appeals it was stated to reference the CMS processing manual chapter 12 section 40.6. This section states that 51 modifier should be attached to the lesser of the two codes. Telemedicine Critical Care Codes

I bill Tele CC 99291,G0508 and G0509 for telemedicine company. I understand that those codes are not covered by Medicaid and Medicaid replacement products (such as Amerigroup etc) But what codes should the doctors be using? or is Critical care just not covered by them? Thank you for your help!

I have my CPC-A. Now I want my CPB. Is this redundant?

In my area, I see a lot of medical billing positions. Way more than coding. I now landed a medical billing job which I start Monday. I graduated from a medical billing and coding program but it was very coding heavy, not so much billing. I feel like CPB would complement my CPC, as well as make me more marketable for future billing positions.

CPB is also one of those certifications I never see anyone have. Is there a point to getting it when you can learn on the job? Also, will I get paid...

I have my CPC-A. Now I want my CPB. Is this redundant?

Accessing practice exams on blackboard

I recently purchased the practice exams for the CIC exam. I am studying this on my own and did not enroll in a course. I logged in to AAPC today to access one of my practice exams and it took me to blackboard. I don't remember every doing anything on there but it prompts me for a username and password. It did send me an email to change my password but I don't know what to do about my username. Does anyone know anything about this or had this same issue?

Signing note≠ Review or verification?

Hello,
I hope that I'm posting this in the correct section.
I am a coder at a physician's practice and recently noticed that one of our providers that uses software to dictate their notes from speech has a note that seems rather problematic, it reads:
"Portions of this note were generated using Dragon Naturally Speaking software. Dictation may not have been thoroughly reviewed."
Some fellow coworkers and myself think this would directly contradict a signature on the note as our understanding...

Application of free (pro bono) skin substitutes for Medicare.

I work for a wound care clinic that applies free samples given to us by pharmaceutical rep to patients. I wanted to know if there's a way to receive reimbursement from Medicare for the application only. If so, how would I bill this? I attended a seminar that stated that modifier "FB" was required to indicate that we are billing the application for a free product. Once billed the claim is denied stating that we used an invalid modifier.

Primary Insurance and Secondary insurance.

If a primary insurance pays telehealth with no modifier, we bill secondary insurance and they deny because no modifier. It is a covered charge, they do require a modifier when billed as primary. Are we required to add the modifier for the secondary claim or should be process and pay with out the modifier based on what the primary did.

Does New Patient E/M require Modifier -25 with injection?

Hello Everyone! :)

I have a quick question, as I am seeing conflicting information. Does a new patient E/M require Modifier -25 when the patient receives an injection on the same day?

Example) New patient is evaluated, diagnosed with right knee osteoarthritis, and receives an injection (methylprednisolone) into his/ right her knee. Does the E/M code need modifier -25?
