Would you code 22600 * 1 and 22614 * 2 or 3 for a C3-C7 cervical fusion? I cannot remember this rule !! Please help, thank you.

Generic name in front of domain name for an employee email

My CFO thinks that using a generic name in front of our secure domain for an employee email is a HIPAA violation . I told him i have not found that information anywhere . "When I see breast cancer patients for follow up, yearly exam am I to use the Z08 after completed treatment for malignant neoplasm? Personal history of breast cancer z85.3 with extra designation to identify side, quadrant, and estrogen status? South Carolina Canceled

This is ridiculous I cannot go to Denver, which was why I chose Charleston, SC. I cannot afford $275 a night nor can I just join virtually that does not cut the mustard. EKG interpretation with ER visit billing question

A patient was seen in the ER, and an ER visit 99284 was billed. An EKG was done that was ordered by the ER doctor, who reviewed it but did not do a formal interpretation. A cardiologist did an interpretation of the same tracing later on the same date. Can the cardiologist's interpretation be billed as 93010? I know if that if the ER doc had done an interpretation it would be included in the ER visit, but if a provider of a different specialty interprets it, can this be billed separately...

Billing for Observation (PCP office)

Question for anyone that can offer some help. We had a new patient yesterday that needed observation after her appt with our physician. She was in our office for about 45min after the appt and the medical assistant checked her blood pressure every 15-20min. The physician is asking if this is something we can bill for but most of the observation codes I see are not used in our type of setting (internal med/pcp). 96130 & 96131

I have a question in mental health billing 96130 & 96131. I know that 96131 is the add on code and I was told by an insurance rep that they must be billed for the same date of service. The office I bill for have one date of service for 96130 and another date of service for 96131, so what is the correct procedure for billing these CPT codes. The techs notes show the two separate dates for the service, therefore if I bill the two CPT for the same date of service is incorrect and fraudulent...

Carrying over CEU's

I was wanting to know if anyone else was interested in seeing if we could carry over some of our extra CEU's that we earn. I will have extra this year and I think we should be allowed to at least carry some over to the next year instead of losing them. We have done the work and paid the money to go to seminars and I don't see why we shouldn't be allowed to carry some of them over.

The more we read the more we are confused about what to code for outpatient hospital telehealth services. Should we use Q3014 or the E&M service? We know how the physician side should bill but cannot locate any clear documentation for the outpatient hospital established patient telehealth coding.

C-Section Assist with Postpartum visit for Infection

Hello everyone,

I have a little bit of a dilemma and I'm hoping that someone can help me out. Here is the scenario:

Patient was seen in the hospital for a c-section and my OB assisted. This patient had previously not been seen in our clinic.
Patient then comes to our office a month later with an infection of the c-section site. She attempted to get into her primary OB but due to COVID was unable to be seen.

We have not billed out the delivery as per our state Medicaid guideline our assist...

holter monitor billing

Hello, I'm looking for help with billing for an Ambulatory EEG. And figured a holter monitor works similary and would be billed similary. My quesiton is, with the POS being in home. What do you put as your location in box 32. (Service Facility Location Information) of the claim form. Seems weird to use the patients address. Would it be billed out under the referring provider or reading physician?

Telehealth documentation

Hello- one of my doctor's is questioning the documentation for his video visits with patients, we are a specialist office seeing patients with audio/video connections. Is the time documentation required? I know for telephone visits it is, but for the video, is time required in the dictation/report? Any help is greatly appreciated.

Abnormal Lab Findings

I work for a lab and on the order it states "elevated potassium" as the Dx. Since I don't have access to the chart notes, I don't know if this was determined based on a previous lab test or not, but I don’t know how else they would know it is elevated without a previous test. I assume it was based on a previous lab test, but we aren’t suppose to assume. Would you query the ordering provider or would you code it to abnormal lab findings? Your opinion is appreciated.

This is my first post on the AAPC forum. I've reviewed the ICD-10 coding guidelines as well as information regarding coding for pediatric preventive care from the AAP. I can't find a clear cut answer to my question. Does an abnormal finding have to be something that's found on the physical exam portion of a well visit in order to qualify as an abnormal exam?

treatment management of SAB using mifiprex and misoprostol

my office recently had a patient who had a non viable seven week pregnancy. she did not want to have a D&C instead opting for treatment using Mifepristone and misoprostol. I can't figure out how to code this. there are plenty of codes when a D&C is done, but none for pharmacological process. ICD10 and CPT codes would be appreciated.
